Greg Collins, PT
FOUNDER/OWNER

Greg Collins is our group's fearless leader who has been serving Naples for nearly 40 years. Greg was born and raised in Chillicothe, OH and relocated to Naples in 1983. Having received his Physical Therapy degree from The Ohio State University, he and his wife opened Suncoast Rehabilitation & Aquatics. Prior to its sale, in 2005, of this start-up company the couple created the largest privately own physical therapy center in Southwest Florida, 8 total clinics. Following a short retirement Greg started Collins Physical Therapy Institute in 2013 with a similar business model focusing on patient care, one-on-one services, and unmatched patient satisfaction.

Greg is a lifelong member and former president, 2004, for the Rotary Club of Naples. Greg prides himself on his involvement with Collier County Organizations such as Big Brother/Big Sister, Quest for Kids, and is a graduate of Leadership Collier in 1998. During his tenure in the physical therapy field he and his wife are founders of three academic and athletic scholarships at Ohio State University and Florida Gulf Coast.


Michael White PT, DPT
CLINICAL DIRECTOR

Michael received his Bachelor's Degree in Health Science from Benedictine University in 2011 and his Doctorate in Physical Therapy from Midwestern University in 2014. Prior to working at Collins PTI, Michael practiced at a Level I trauma center in Downtown Chicago, which was affiliated with top rated rehabilitation hospital RIC (now Shirley Ryan Ability Lab).

Michael is currently progressing towards his mechanical diagnosis certification through the Mckenzie Institute USA.


Carrie Fulton PTA, LMT, GTS
PHYSICAL THERAPY ASSISTANT

Carrie is originally from Upstate New York and obtained her PTA degree from Chesapeake College in Annapolis, MD in 2015. She received her COMPT manual certification from Florida Institute of Manual Therapy in 2016. Carrie has 15 years of experience as a Licensed Massage Therapist and is also a Graston Technique Specialist (GTS).


Jackie Augustine
OFFICE MANAGER

Jackie has been with Collins Physical Therapy Institute since 2014 serving in a variety of roles, working her way through the ranks to become our Office Manager. She oversees the daily administrative operations at both clinics. She also oversees the billing and accounts receivables for the organization. Jackie prides herself on delivering exceptional customer service with a warm and caring attitude.


Nicole Rivera
PTA, GTS

Nickie is originally from Daytona Beach, FL and received her Bachelor's Degree in Exercise Science from Ave Maria University in 2015. In 2017 she received her PTA degree from Hodges University in Naples, FL. Nicole is excited to develop her skill set with Collins PTI. She prides herself on patient first care and compassion for her patients. Nicole is a Graston Technique Specialist (GTS).

 


Kristina Clinton PT, DPT
PHYSICAL THERAPIST

Kristina received her Bachelor's Degree in Kinesiology at Penn State University in 2013, and her Doctorate in Physical Therapy from Shenandoah University in 2017. Since then she has worked as an outpatient physical therapist, and has had the opportunity to treat a wide array of musculoskeletal conditions throughout the human body. At this time she is continuing to expand upon her clinical skills by pursuing certification through the McKenzie Institute.


Kiran Mahadik PT, DPT, OCS
PHYSICAL THERAPIST

Kiran (key-run) Mahadik comes with experience of 14 years in varied settings and specialties. His education background is Doctorate in Physical Therapy from EIM with a Manual therapy certification. He is board certified specialist in orthopedic physical therapy and believes in evidence-based approach to treat his patients. He also has a Master's degree in sports PT with exercise physiology as a major. Kiran is originally from India and enjoys the warm weather Florida offers. He likes hiking, biking and rock climbing.


Luke Witmer PT,DPT
CLINICAL DIRECTOR

Luke graduated from Florida State University in 2012 with a Bachelor's degree in Exercise Science and from University of North Florida in 2016 with his Doctorate in Physical Therapy. Luke sought out specialized training in sports medicine and orthopedic rehabilitation at Texas Metroplex Institute, home of Texas Rangers Baseball, and at the University of Florida Orthopaedic and Sports Medicine Institute, home of Gator athletics. Luke has work experience in outpatient, home health and skilled nursing settings. Luke has practiced physical therapy for over five years and successfully rehabilitated over a thousand patients who have undergone surgeries, suffered injuries, strokes, as well as movement and balance disorders of all kinds. Luke is passionate about helping others and strives to assist his patients not only in their rehabilitation but also in navigating difficult healthcare decisions with a patient-first mentality. In his free time Luke enjoys spending time outdoors, disc golfing, exercising, playing guitar and spending quality time with family and friends.

 


Kelly Nelson
ASSISTANT OFFICE MANAGER

Kelly received her degree in Marketing from Florida State University (FSU). She completed a 2-year program at FSU to obtain her certification in Public Management and attended the University of South Florida for social marketing certifications. A native Floridian, she worked for the state health department for 17 years, specifically in emergency response and preparedness. Kelly works in Insurance and Verification at Collins PTI. She is a breast cancer survivor and proudly shares her story of survival with patients. She enjoys working with patients and prides herself on providing excellent customer service.

Ashley Torres

Ashley was born and raised in Naples, FL. She has been working in the medical field for over 6 years, motivated every day to help patients get the best care with a smile on the side! She is typically the first person you come in contact with over the phone and in the office at our Commons location. She prides herself on getting patients scheduled within 24hrs!!

She works hard to create a positive, loving and caring environment in the Commons Office.
